For people in Stretford, Sale and Sale Moor who are ready to reconnect with themselves, rebuild confidence, and begin moving towards what comes next.

Living with chronic pain, fatigue, anxiety or a long-term health condition can slowly make your world smaller. You might have stepped away from work. Lost confidence in what you're capable of. Become more isolated. Or simply feel as though your health has been making the decisions for you.

Breaking Beyond Diagnosis is about beginning to change that.

The starting point

This isn't about fixing you.

It's about helping you rediscoverwhat's already there.

When you've spent months or years managing symptoms, appointments, responsibilities and uncertainty — or simply trying to get through the day — it's easy to lose sight of the person underneath it all.

Your confidence can change. Your relationships can change. Your connection with the outside world can change. And sometimes even imagining employment, education, volunteering or trying something new can feel enormous.

That's why we don't start there. We start with you.

Over 16 weeks you'll have space to understand what's been holding you back, build practical tools to support yourself, reconnect with other people, and gradually explore what your own next chapter could look like.

There is no one-size-fits-all destination. Your progress might look completely different from somebody else's — and that's exactly how the programme is designed.

Who’s behind this

I’ve been where you are.

“I know what it’s like when your health starts making the decisions.”

I’m Reanne, founder of The Phoenix Movement CIC. Before any of this existed, I was someone sitting with a fibromyalgia diagnosis, wondering what my life was going to look like now.

I know the exhaustion that sleep doesn’t touch. I know what it is to cancel the things you’d been looking forward to. To feel your world getting quieter and smaller, and to wonder whether this is simply how it is from here.

It isn’t.

What changed things for me wasn’t one big breakthrough. It was understanding how my nervous system actually worked. Finding tools that fitted around my energy instead of fighting it. And slowly — genuinely slowly — building back a life that felt like mine again.

I won’t tell you I found a secret nobody else has. What I will tell you is that I live very differently now than I did then, and everything I learned getting here is what this programme is built from. Not from a textbook — from living it, and then training properly in the things that helped, so I could walk it alongside other people.

I’m not here to fix you. I’m here to sit alongside you while you work out what’s possible.

This is not 16 weeks of somebody telling you what you should be doing. We'll meet you where you are.

The programme combines online support, in-person experiences, group connection and individualised support, so you build from your own starting point. You'll be encouraged to take manageable steps rather than giant leaps.

Not by pushing you.

Not by expecting you to suddenly “get back out there.” And definitely not by treating everyone as though they need the same thing.


As the weeks progress, we begin to look beyond simply managing today, towards a bigger question:

The shape of the journey is the same for everyone. What you do inside it isn't.

Phase one — Reconnect

Understanding where you are now

We begin gently.

You'll meet the group, explore where you currently are in life, and begin understanding some of the patterns that may be influencing your confidence, energy, stress and wellbeing. You'll be introduced to practical approaches around nervous-system awareness, self-management, and recognising what your body may need.

The aim: greater understanding, connection and hope.

Phase two — Rebuild

Developing tools that work for you

This is where we begin building.

Through online sessions, coaching, workshops and practical exercises, you'll explore tools that help you navigate challenges differently. We may look at confidence, habits, boundaries, stress, self-belief, emotional wellbeing, communication, and reconnecting with your own strengths.

The aim: to feel increasingly equipped rather than powerless.

Phase three — Expand

Taking your world beyond its current limits

As confidence grows, we begin looking outwards.

What interests you? What strengths have you forgotten about? What would you love to try? What feels realistic now that perhaps didn't at the beginning? This might mean reconnecting with your community, learning something new, volunteering, exploring opportunities — or simply doing something independently that previously felt out of reach.

The aim: possibility.

Phase four — Your next chapter

Turning progress into something sustainable

The programme doesn't end with us simply saying goodbye.

We'll help you recognise how far you've come and create an individual progression plan for what happens next. Depending on your goals, that could include connections into community opportunities, volunteering, adult learning, further support, employment services or enterprise.

The aim: you leave knowing your next steps — and believing you're capable of taking them.

Online and in person

Built around the reality of a health condition.


We know rigid programmes can be difficult when your energy isn't predictable. Some sessions take place online, so you can take part from your own space. Others happen in person, creating room for genuine connection, community and new experiences.

You won't be expected to navigate the journey alone.

Something else important

We're keeping the group small — on purpose.

We don't want you disappearing into a room full of people, or becoming another number on a spreadsheet. We want to know your name. We want to understand what matters to you.

And we want enough space in the programme to recognise that 15 people can arrive with 15 completely different stories.

Somebody may want to return to employment. Someone else might want enough confidence to volunteer. Another person may simply want to stop feeling so isolated and begin reconnecting with their community.

All of those beginnings matter.

After 16 weeks

We don't expect you to become a completely different person.

We want you to feel more like yourself again. By the end, our aim is for you to leave with:

  • Greater confidence in yourself and your abilities
  • A better understanding of your nervous system, stress and wellbeing
  • Practical tools you can keep using long after the programme ends
  • Greater connection with people and your local community
  • Reduced feelings of isolation
  • Stronger self-belief about what you're capable of
  • Greater clarity about what you want next
  • An individual progression plan with realistic next steps
  • A renewed sense of hope for your future

And perhaps most importantly — evidence to yourself that your diagnosis doesn't get to write the rest of your story.

You don't need to have everything figured out. And you certainly don't need to know what you want to do for the rest of your life.

What we're looking for isn't perfection. We're looking for willingness. A willingness to show up. To explore. To try something different. And to commit to the 16 weeks alongside us.

  • Chronic pain, a long-term health condition, fatigue, anxiety, low confidence, isolation or other life circumstances have made it harder to move towards employment or wider opportunities

Before you apply

This is a 16-week commitment.

The programme is fully funded, which means there's no financial cost to eligible participants. But there is one thing we ask you to invest: your commitment.

Places are limited, and we want them to go to people who genuinely want to take part. You'll be joining both online and in-person elements across the programme, so please consider whether you're able and willing to commit to the journey before applying.

You don't have to arrive feeling fearless. You just have to be willing to begin.
